How to Access Zest4Living Services
STEP 1: Referral
GP to complete an Enhanced Primary Care (EPC) Program Referral for eligible individuals to access Physiotherapy and/or Occupational Therapy Services under Medicare.
OR
Medical Practitioner to complete a referral to Physiotherapy or Occupational Therapy Services using a DVA Request/Referral D904 on their own letterhead for DVA patients. This should state what services and how many sessions are required.
OR
Individuals without an EPC Plan can self-refer if they are planning to claim from their ‘extras’ private health insurance. Individuals are encouraged to consult their health fund regarding their eligibility prior to attending Zest4Living Therapy Services.
STEP 2: Book Appointment
Zest4Living Therapy Services will contact each patient regarding an appointment time on receipt of medical referral.
OR
Individuals can contact Zest4Living Services directly to arrange an appointment once they have consulted their private health fund.
STEP 3: Centre-Based Therapy
Zest4 Living Therapy sessions take place in the John Flynn Respiratory Centre and a report is then sent to the GP regarding the individual’s progress and outcome.
